Complete this form as

to us, we will check it

be replaced or a refund

shipping box and packaging
materials ensuring that your
product is wrapped securely.

name and address and the
order number so we can
identify the parcel on
receipt. Please ensure you
get Proof of Delivery
and Proof of Postage for
your return, failure to have
a POD could result in you
not receiving a refund.
